Engaging patients in a therapeutic partnership to empower and inspire change
Consultations are conducted by a trained nutrition professional and recommendations are made based on the principles of human clinical nutrition from a functional medicine perspective.
Clients will receive personalized care and recommendations based on the type of service purchased, health status, and personal goals. Success in any program may require the client to adopt significant changes to diet, lifestyle, stress management, and exercise practices.
At this time, appointments are only available via HIPAA compliant telehealth video-conferencing and telephone.
All clients will be required to have a laptop, computer, smartphone or tablet as well as internet access in order to participate in telehealth consultations.
New Client Consultations
Clients may choose from several appointment package types. However, all new cleints will be required to complete waivers, agreements, intake questionnaires, lifestyle journals, and symptoms questionnaires.
Follow-up Consultations
Follow-up consultations are a critical component to client success. Dietary and lifestyle changes require motivation, time, and support so that these changes can be adopted for a lifetime. Follow-up appointments serve to fine-tune and change treatment plans over time to meet changing needs along the journey to optimal health.
Hours of Operation
Hours may vary from week to week, but generally, appointments are available during normal business hours Monday- Thursday with new patient visits being scheduled on Mondays.
Payment
Insurance is not accepted, though some may qualify for reimbursement. It is the responsibility of the payer to determine if reimbursement is an option. If so, requests for superbills must be made 1-2 weeks in advance and will require 1-2 weeks to complete. While some HSA/FSA credit cards may be accepted for services, it is the client’s responsibility to determine if your HSA/FSA will cover nutrition consultations. Regardless of reimbursement, payment is due in full when the service is booked and/or conducted. If payment is made in advance and session canceled 24 hours before the scheduled appointment time, refunds will be given/ issued.
Cancellations/missed appointments
Nutrition counseling using a functional medicine approach is very time intensive. In order to ensure that access to care is available for all in need, this practice maintains a strict 24-hour cancelation policy. This policy requires notice of 24- business hours for cancellation of all appointments. If cancellations are not made 24 business hours prior to the appointment time, full fee will be charged. If no cancelation is made, it will be considered a No-show and will be charged using the credit card on file.
Email or Messaging in-between appointments
The preferred means of communication is through the chat feature in the patient portal. Emails will be answered within 48 business hours Monday-Thursday. Emails sent Friday- Sunday may have slower response time. If questions require lengthy responses or are very detailed, a recommendation will be made for a follow-up appointment to address needs more appropriately.
Medical Records
In compliance with HIPAA, medical records can only be released with written consent. Clients are responsible for obtaining medical records for appointments Once attained, records should be mailed or faxed at least one week prior to the first appointment. In the case of going commutation with other providers, release forms may be given.
Intake Forms
Danielle’s intake forms are long and detailed and will take some time to complete. Please allow sufficient time to complete forms. Forms not completed prior to the initial visit may deduct time during the initial appointment for completion and may trigger the need for additional follow-up appointments. Forms can be completed online using the patient portal. Alternately, PDF versions are can be provided upon request and then be printed, completed by hand, and uploaded. Because the this is the basis of any Functional Nutrition Plan Regardless of how forms are completed, it is important to answer all questions with detail and accuracy.
As part of the forms, You will also be asked to include in the required forms is a 3-day diet and lifestyle diary. Please eat normally during this time.
All clients are required to provide baseline bloodwork for any comprehensive and personalized functional medicine nutrition services. Specific details will be provided when joining as a member through the patient portal.
All intake forms and submitted medical records/labs must be completed or received at least one week prior to the first appointment. If this is not done, the initial appointment may need to be rescheduled to a later date.
